Introduction

These Terms of Service (“Terms”) govern your use of massage therapy services offered by A Massage By Nancy Massage Therapy (“I”, “me”, or “us”). By booking an appointment or receiving services, you agree to be bound by these Terms.

Appointment Booking and Cancellation

• Booking: Appointments can be booked by phone, email, or online scheduling system (if applicable).
• Cancellation: We require a minimum of 24 hours notice to cancel an appointment without a fee. Cancellations with less notice or missed appointments may be subject to a cancellation fee of $25.00 for less than 24 hours’ notice or the full amount of service if less than 4 hours’ notice

Payment

• Payment is due at the time of service. We accept
• We reserve the right to request a credit card on file to secure your appointment.
• Gratuities are welcome but not mandatory.

Client Conduct

• You agree to arrive for your appointment on time and to disclose any relevant medical conditions or concerns before the massage begins.
• Massage therapy is intended for therapeutic or relaxation purposes only. Any inappropriate behavior will result in termination of the massage and you will be responsible for the full service fee.
• Sexual harassment of any kind will not be tolerated and will be reported to the authorities.

Scope of Services

• I am a licensed massage therapist and provide massage services within the scope of my practice.
• Massage therapy is not a substitute for medical care. I cannot diagnose or treat medical conditions, nor prescribe medication.
• If you have any concerns about your health, you should consult with a physician.

Client Responsibility

• You are responsible for informing me of any changes in your health conditions between appointments.
• You are responsible for communicating your comfort level throughout the massage and requesting adjustments as needed.

Disclaimer of Warranties

Massage therapy services are provided “as is” and without warranties of any kind, express or implied. I do not warrant the outcome of any massage therapy service.

Limitation of Liability

I shall not be liable for any damages arising from or related to massage therapy services, including but not limited to, personal injury, property damage, or economic loss.

Termination

I reserve the right to refuse service to anyone for any reason.

Entire Agreement

These Terms constitute the entire agreement between you and me regarding massage therapy services.

Governing Law

These Terms shall be governed by and construed in accordance with the laws of the State of [Your State].

Severability
